Room & Care is partnered with Washington State Department of Social and Health Services (DSHS) and Washington's Area Agencies on Aging (AAA) to help Medicaid Case Managers find real-time Adult Family Homes and Assisted Living availability for client referrals.
Medicaid Case Managers use Room & Care's proprietary referral technology to match clients with providers based on their care needs, preferences, and each provider's capabilities.
Medicaid-contracted Adult Family Homes and Assisted Living Facilities can list their Medicaid openings on Room & Care for free. Providers identify the type of room available along with 20+ resident characteristics to help identify clients who fit their capabilities, limitations, and areas of specialized care.
Case Managers upload the client's CARE Assessment, and enter the client's preferred city or neighborhood and other personal preferences. Room & Care then identifies available providers whose openings and capabilities match all of the client's needs.
Case Managers securely send referral and assessment information to matching providers. Providers review the client's preferences and care needs, and indicate whether they are able and willing to accept the client.
The client or family receives a list of providers that have accepted the referral. They can compare photos, services, amenities, and other facility information, contact providers directly, and schedule in-person tours before selecting the home that's right for them.
Eligibility for Medicaid long-term care services is based on both financial and functional requirements.
