AVALON CARE CENTER PORTLAND is a warm and inviting Nursing and Residential Care community for those in need of intermediate and long-term care.

We are committed to nurturing an environment where residents and their loved ones can feel at home. Located in a quiet neighborhood, our warm, welcoming community is filled with natural lighting that pours through our skylights and spacious windows. Private and semi-private rooms look out onto the secured courtyard that is complete with a gazebo and gardening areas. Our living spaces feature three dining rooms, various activity areas, and a variety of relaxed settings and spaces that are perfect for leisure and social interaction.

Long Term Care

Our skilled nursing staff is on call 24 hours a day and can help your loved one live safely and comfortably, in our care. We also offer a wide variety of services designed to meet physical, emotional, and social needs of those with special conditions. Your loved one will receive the individualized care and attention they need to remain happy, healthy, and safe in their new home.

Memory Care

Residents needing memory care thrive at Avalon Care Center Portland. Our secured community affords residents with a wide range of opportunities for engagements and activities that improve their cognitive and mobility functions and gives a boost to their quality of life. We have years of experience caring for residents who are living with the symptoms of dementia, including wandering behaviors. Our problem-solving approach to providing dementia care has proven effective in developing interventions that are specific to each resident’s needs.

At Avalon Care Center Portland, we believe that activities are an integral part of a care community. Whether your loved one prefers activities independently or taking part in one of our organized events, we have something for everyone. Our activities teams offer a diverse range of programs, including a garden group, exercise class, sensory activities, cooking class, musical entertainment, and various other special events. We also promote individualized activities that your loved one can enjoy independently, such as reading, puzzles, and art projects.
