Creekside Place, a memory care community is part of one of the fastest-growing cities in the state of Washington, Battle Ground! Located just off Highway 502 in Battle Ground, Creekside Place has been open and providing care for Residents since the late 1990s. Caring Places Management is honored to carry on the tradition of quality care in a homelike environment that Creekside Place has been known for in the Battle Ground and the surrounding area since its opening.

Creekside Place, a memory care community, is a wonderful single-story building featuring 38 Apartments, with 8 shared apartments and 30 large studio apartments, each with its own bathroom including a roll-in accessible shower. The building encloses an interior courtyard with access to a walking path, gardening, and covered seating areas. The community hosts a bright and open dining room and home-like living room, an activity room, and access to non-pay laundry if desired.

The team at Creekside Place prides itself on the quality and caring manner in which they work with the Residents and serve the greater community. Our robust activity and life enrichment programming are geared to resident preferences and include visits by music and other entertainers, Bingo, scheduled trips to stores and other desired locations, wonderful scenic drives, and enjoyment of all the area has to offer!

Caring Places Management is proud to be working to provide Medicaid-funded services for appropriate residents as possible based on availability and other factors at Creekside Place. More Information About Creekside Place

Creekside Place is a Memory Care facility located in Battle Ground, WA. In Washington State, Memory Care is a term used to describe the professional care provided to individuals who suffer from Alzheimer’s or other types of dementia. Memory Care in WA can typically be found across 3 primary types of assisted living – Adult Family Homes, Assisted Living Facilities, and Nursing Homes. Memory Care is normally considered an Add-On service in addition to the daily assistance with Activities of Daily Living (ADLs) required by the resident. Aside from the specialized training and certifications required for staff and caregivers, the buildings themselves must be equipped with certain features such as special locks on the doors before they can be licensed for Memory Care. Creekside Place is fully certified, and up to date on their training and certifications to properly care for residents with Alzheimer’s/dementia.
Creekside Place currently has a room available, and is looking for the right resident to join their home. The available room listed here is shared, meaning you will have a roommate. The bathroom is private, and wont be used by anybody else but you.

Pets:

Creekside Place does not allow pets. This does not necessarily mean there are no pets in the facility, but that they do not allow residents to bring their own.

Smoking Policy:

Creekside Place does not allow smoking on the premises, but they do allow the use of Vapor/E-cigarettes in designated areas. They have one or more designated areas outside where Vapor/E-cigarette users can vape away from the building and away from other residents. Vaping indoors or anywhere outside the designated areas is strictly prohibited.

Short-Term / Respite Care:

Creekside Place offers Short-Term / Respite Care with daily rates for anyone who needs to stay for less than a month. Respite Care services can offer relief for current caregivers, allowing them to take a much-needed break.

Hospice Care:

They provide personalized and compassionate hospice care services for residents and their families during the end-stages of life. Respect and dignity are top priorities for residents and their families – especially during these times.

Seeking Medicaid Resident

Creekside Place accepts Medicaid, and is seeking a Medicaid resident for this available room. Medicaid recipients typically need to have an assessment from Medicaid which will include the resident’s level of required care, as well as the daily and monthly dollar amount that Medicaid will cover for their care. There may also be certain waivers available for qualifying residents, which may cover additional items that Medicaid itself will not cover. Each state has very different Medicaid rules, coverages and limits, as well as different available waiver programs. If you have not already done so, please contact your local Medicaid office to determine what you need to do in order to get an official assessment, and if you qualify for any additional coverage. Once you have that, you can contact this facility (or any other facility that accepts Medicaid) and they will ask you some questions about the care you require and about your Medicaid assessment. The facility will make the final determination whether to accept a resident or not. Room & Care encourages residents to contact multiple facilities to improve their odds of being accepted faster. Facilities look at factors such as whether the resident will fit in with their other current residents, if the resident requires a similar level of care as the other residents in the facility, current number of available caregivers or a lack thereof - among other factors.

What's included?

Creekside Place provides 24/7 supervision, emergency assistance and daily companionship as a base service for all residents.

Meals

Breakfast, lunch and dinner are included every day. Accommodations can be made for any dietary restrictions or special doctor-prescribed diets. Additional food items such as coffee, tea and snacks between meals are also included.

Medications Management

Medications Management is provided as a service for all residents. Caregivers and staff keep track of administration schedules as well as prescription refill schedules.

Blood Pressure and Weight Checks

Regular blood pressure and weight checks are done at scheduled intervals, and recorded to track irregularities.

Housekeeping and Laundry

Creekside Place does all daily housekeeping throughout the home as well as in your room. Laundry is provided for your flat linens on a set schedule or sooner if needed. Your personal laundry is also washed and folded regularly by care staff.

Activities

A range of social and recreational activities are offered every week to keep residents occupied, entertained and engaged. Feel free to ask what types of activities are scheduled for this week.

Utilities

All standard home utilities are included in the base price. (This excludes personal cell phone plans which are paid for by the resident or family.)

Transportation

Local transportation for certain scheduled outings, as well as doctor visits is provided at no additional cost.
